A) Yes, in order to insure being seen by a practitioner in a timely manner it is important to schedule an appointment in advance.


Q) What do I need for my first appointment?

A) a: Arrive 15 min before appointment, *you may print your patient forms from the link at the bottom and right of this page, fill them out and bring them to save time at your first appointment.

    b: A prescription from your referring physician

    c: Current insurance cards


Q) I received an EOB/letter from my insurance company stating they need additional information, what should I do?

A) Our office may not receive your EOB or a letter from your insurance company as quickly as you do. It may take as long as a week longer. Our hands are tied until we receive the request directly from your insurance company.
